Lodging a complaint

Overview

You can make a complaint to us or to the Health Care Complaints Commission.

You need to make your complaint in writing. You can make a complaint using our online complaints form or by downloading and completing complaints from which you can send to us by post or email.

It is helpful if you include as much information as possible in your complaint, such as the name of the practitioner, their profession and any other information about what happened. If you need help putting your complaint in writing you can call us on 1300 197 177.
